Using Documentation Guidelines: Applications to Clinical Service

Abstract

Documentation is an essential element in the provision of support services for students with learning disabilities. It assists in the two-fold process of determining whether a student is eligible for accommodations and of determining what accommodations are appropriate for the student in specific academic settings. Documentation guidelines have been developed to provide service providers, diagnosticians, and students with a common foundation outlining the components of documentation needed to validate a disability and upon which to make infonned decisions about appropriate accommodations. This paper will address the application and potential misuses of documentation guidelines.
